What Are Pap Smear and Liquid-Based Cytology Tests?
A Pap smear test and liquid-based cytology (LBC) are cervical screening methods used to detect abnormal cells in the cervix before they develop into cervical cancer.
A Pap smear involves placing cervical cells directly onto a slide
Liquid-based cytology (LBC) places the sample into a liquid medium for improved accuracy
At Evergreen Medical Clinics, we use modern screening methods to ensure more reliable and accurate results for our patients.
Why Cervical Screening Is Important
Cervical cancer often develops without early symptoms. Regular screening helps detect:
Early HPV-related changes
Precancerous cervical abnormalities
Signs of cervical cancer at an early, treatable stage
Understanding Your Pap Smear or LBC Results
1. Normal Result (NILM)
Negative for Intraepithelial Lesion or Malignancy (NILM)
No abnormal cells detected
Indicates a healthy cervix
Recommended action: Continue routine screening every 3–5 years based on your doctor’s advice.
2. ASC-US (Atypical Squamous Cells of Undetermined Significance)
Mild, unclear cell changes
Commonly caused by HPV infection
Recommended action:
Repeat Pap smear, or
HPV DNA testing
3. LSIL (Low-Grade Squamous Intraepithelial Lesion)
Mild abnormalities linked to HPV
Often resolves naturally
Recommended action:
Monitoring or repeat testing
Possible colposcopy if needed
4. HSIL (High-Grade Squamous Intraepithelial Lesion)
More serious abnormal cells
Higher risk of developing cervical cancer
Recommended action:
Immediate further testing
Possible treatment to remove abnormal cells
5. Atypical Glandular Cells (AGC)
Less common but more concerning
May indicate deeper abnormalities
Recommended action: Further investigation is required.
6. Cancer Cells Detected
Possible cervical cancer diagnosis
Recommended action: Urgent referral to a Gynaecologist specialist for treatment.
What Makes Liquid-Based Cytology (LBC) Better?
Liquid-based cytology offers several advantages over conventional Pap smears:
Higher accuracy in detecting abnormal cells
Fewer inconclusive results
Cleaner, clearer samples
Ability to perform HPV testing from the same sample
This is why we, at Evergreen Medical Clinics, prefer LBC for cervical screening.
What Does an “Unsatisfactory” Result Mean?
An unsatisfactory result means the sample could not be properly analyzed.
This may happen due to:
Insufficient cells
Blood or mucus interference
Next step: A repeat test is required. This is common and not a cause for concern.
Do Abnormal Results Mean Cancer?
No—most abnormal Pap smear or LBC results do not mean cancer.
In fact:
Many mild abnormalities resolve on their own
Cervical cancer typically develops slowly over several years
Regular follow-up ensures any changes are managed early.
When Should You Get a Pap Smear in Malaysia?
General guidelines:
Ages 21–29: Pap smear every 3 years
Ages 30–65: Pap smear + HPV test every 5 years
If you’re unsure, consult a doctor for a personalised screening schedule.
